AMH receives most improved performance award in ACO Program

On April 28, Antelope Memorial Hospital received the Most Improved Performance Award for 2025, as a member of the Accountable Care Organization coordinated by Bryan Health. The award was presented during the annual Bryan Health Connect Conference held at the Holthus Convention Center in York.

An Accountable Care Organization in Nebraska is a group of doctors, hospitals and healthcare providers who voluntarily work together to provide high-quality, coordinated care to patients. These networks aim to lower costs for the patient and improve health outcomes, primarily by focusing on value-based care rather than volume-based payments. A key purpose of an ACO is to prevent medical errors, reduce unnecessary duplicate tests and to improve patient health.

The 2025 ACO performance awards were based on ACO participants' performance in key quality measures. Annual wellness visit completion, breast cancer screening, controlling high blood pressure and glycemic status for diabetic patients.

Antelope Memorial hospital, which includes all AMH Family Practice Clinics, was recognized after demonstrating the most improvement in the key quality measures of ACO from 2024 to 2025. AMH's staff had dedicated time and effort to modify workflows and to focus on each of their contracts, resulting in a noticeable improvement in their quality scores.

"Here at AMH, we emphasize annual wellness visits, timely health screenings and proactive chronic disease management," said Tanya Sharp, chief clinical operations officer at AMH Family Practice Clinics. "This focus serves to improve patient health, reduce the need for emergency care and hospitalizations and lower long-term costs for the patient. AMH staff strives to put the patient at the center of their care and to help him/her to navigate a complex medical system. We help manage chronic diseases, coordinate between different doctors or members of the care team and provide more preventive health services to keep our patients healthy. Plus, additional recovery support is provided when they come home from the hospital as well as more convenient avenues for receiving care such as home-based care, telehealth or other virtual means."

AMH Family Practice Clinics has locations in Elgin, Clearwater, Orchard, Tilden and Neligh. For appointment with an AMH provider, call 402-887-5440.
